To make school and sports physicals more convenient and accessible for our patients, Faith Regional Physician Services Wayne Family Medicine will offer physical appointments after hours this summer. Call (402) 375-2500 to schedule an appointment with one of our providers between 4:00 PM to 6:00 PM on one of the following dates: Thursday, July 11, Thursday, July 18 & Thursday, July 25. You may also schedule an appointment for your child during our regular clinic hours of Monday - Friday between 8:00 AM - 4:00 PM. School physicals are $75.00, and sports physicals are $35. If not sent to insurance, you must pay this fee at the time of the visit. Please complete the school or sports physical form provided by the school before your child's appointment and have your child ready to provide a urine sample in the clinic or bring a sample with you to their appointment.
